Topic: Successfully Transitioning Survivors From Treatment to the “New Normal” – What You Can Do To Make A Difference.

During this presentation participants will:
• Learn how to address the common emotional issues patients face when treatment ends
• Receive specific tools to help patients transition from treatment to a value based, empowered way of living
• Gain new psychosocial resources and services for patient referral
