The Oncology Social Worker will work as an integral member of the interdisciplinary health care team and is responsible for addressing the psychosocial needs of patients and their families, as well as coordinating the services to maximize a patient/family’s quality of life. The Social Worker’s interventions take into consideration the patient’s strengths, resources, insurance coverage, family system, and motivation for change. In collaboration with the multidisciplinary oncology team members, the Social Worker is responsible for identifying high-risk patients through screening mechanisms; performing assessments of patient/families to determine and facilitate the most appropriate intervention; and providing support and intervention for the social and emotional needs of patients and their families to promote continuity of care and services. The main focus of the Social Worker is psycho-social assessment, mental health evaluation and referral, facilitation of support groups, patient advocacy, aftercare planning, information/referral and outpatient follow-up.
The selected candidate will possess a Master’s degree and a minimum three years post-Master’s degree experience. Clinical Social Work Licensure (LCSW) by the State of Florida is required. Any appropriate combination of education, certifications and/or relevant work experience will be considered.
